Print the official SoonerCare Doula Services Referral Form and bring it to your prenatal appointment.
Your OB, midwife, physician, physician assistant, APRN, or licensed midwife can complete the referral.
The form instructions say to mail or fax the original to the provider you are referring to. Keep a copy for your medical chart.

What the benefit covers
